Medicare GLP-1 Bridge Program: What Agents Need to Know

In short: The Medicare GLP-1 Bridge is a CMS program that runs from July 1, 2026, through December 31, 2027. Eligible clients with a Part D plan or a Medicare Advantage plan with drug coverage can get certain GLP-1 weight-loss drugs, including Wegovy, Zepbound and Foundayo, for a $50 copay per fill. Clients must meet BMI and health requirements, and their doctor must get prior authorization. Agents help by explaining the rules and making sure clients keep the drug coverage they need.

Few topics drive more client calls right now than GLP-1 drugs. Until now, Medicare couldn’t pay for these drugs when they were used only for weight loss. The Bridge program changes that for clients who qualify.

Clients will hear about it on the news and from friends. The agent who can explain it clearly, and correct the myths, earns trust fast.

How Does the GLP-1 Bridge Program Work?

  • Dates: July 1, 2026, through December 31, 2027
  • Where: all states and territories
  • Cost to the client: $50 per covered fill, the same in every phase of the Part D benefit
  • Covered drugs: Wegovy (weekly shot or daily pill), Zepbound KwikPen and Foundayo, when prescribed for weight management. The list may grow during the program.
  • What it doesn’t do: the $50 copay doesn’t count toward the Part D deductible or the yearly out-of-pocket cap, and manufacturer coupons can’t be used with it

The prescription must be for the exact covered version of the drug to get the $50 price.

Who Qualifies for the GLP-1 Bridge?

A client must be enrolled in a stand-alone Part D plan or a Medicare Advantage plan with drug coverage. Their doctor must confirm they meet one of these BMI rules:

BMIAlso required
35 or higherNothing else
30 or higherHeart failure, uncontrolled high blood pressure or chronic kidney disease
27 or higherA prior heart attack or stroke, prediabetes or peripheral artery disease

CMS looks at BMI from when the client first started the drug. Prior weight loss on a GLP-1 may not disqualify them.

Clients using these drugs for type 2 diabetes, sleep apnea or certain other conditions are covered through regular Part D, not the Bridge.

How Should I Talk With Clients About GLP-1 Coverage?

  1. Send them to their doctor first. The doctor decides eligibility and submits the prior authorization.
  2. Don’t promise approval. Explain the rules, but let the doctor and plan make the call.
  3. Set expectations on timing. High demand may mean delays in the early months.
  4. Correct the coupon myth. Discount cards and manufacturer coupons don’t work with the Bridge.
  5. Protect their drug coverage. A client without Part D or MA drug coverage can’t use the program. That makes drug coverage a key part of every plan review.

This is also a good time to remind clients that Medicare-negotiated prices for Ozempic, Rybelsus and Wegovy start January 1, 2027. See our post on Medicare Part D changes for 2027.

What Does This Mean for My AEP Reviews?

GLP-1 questions will come up in many AEP appointments. Before AEP, make sure you can:

  • Explain the difference between Bridge coverage and regular Part D coverage
  • Check each client’s other drugs against plan formularies
  • Document the conversation in your CRM

Our AEP client review checklist walks through the full review.
